Duval County Death Certificate Services

Death certificates are available for deaths occurring in Florida from 2009 to present. Any death prior to 2009 must be requested from the Bureau of Vital Statistics in Jacksonville or through VitalChek.

Duval County issues two types of death certificates. The public version without cause of death is available to any person aged 18 or older. This certificate shows the decedent's name, date of death, and other vital information. It meets most legal and financial needs.

The confidential version includes cause of death. Florida law restricts access for 50 years. Only the spouse, parent, child, grandchild, or sibling may obtain this type. Others must provide documentation of interest in the estate.

Fees are $15 for the first death certificate and $10 for each additional certificate. These fees are set by the Duval County Health Department.
